This revision corrects the Unit 2 information previously submitted on April 10, 1992.

Unit 11 There were three challenges to the pressurizer safety and power operated relief valves (PORV) at Unit 1

during the period of January 1, 1991, to December 31, 1991.

April 13, 1991 Plant Conditions:

Mode 4, RCS pressure was 300 p31, Overpres-9 sure Protection System (OPPS) was in service.

A cooldown to Mode 5 was in progress.

Summary:

Pressurizer Power Operated Relief Valve, PCV-RC-445C, opened while OPPS was in service due to RCS pressure slowly reaching 327 psig.

Pressure decreased to 303 psig and PORV reclosed.

July 4, 1991 Plant Conditions:

Mode 4, RCS pressure was 315 psig, Overpres-sure Protection System (OPPS) was in service.

A cooldown to Mode 5 was in progress.

Summary:

Pressurizer Power-Operated Relief Valve, PCV-RC-445C, opened for 37 sect ils then reclosed, while reactor operator was controlling RCS pressure in a narrow band to maintain RC-P-1C

Mode 5,

RCS pressure was 360 psig, Overpres-sure Protection System (OPPS) was in service.

Summary:

Pressurizer Power Operated Relief Valve, PCV-RC-455C, opened for 10 seconds then reclosed.

The cause for actuation was unknown.

No over-pressure condition existed.

Mode 1

at 100 percent reactor power, RCS pressure and temperature at normal operating values.

Summary:

A reactor / turbine trip occurred due to closure of three of four governor valves.

2RCS*PCV455C opened for 1.546 seconds then reclosed.

Tne cause for actuation

. was the primary system pressure increase following the load rejection.
