Vibration and Loose-Parts Monitoring System r
i On November 20, 1995, Loose Parts Monitoring System (LPMS) Channel 5
-(Upper Rx Vessel at 120 Deg) was declared inoperable due to erratic Preamp
[
Bias Voltage indications.
This was discovered during periodic surveillance 1 testing per PT/0/B/4600/03.
The alarming capability of Channel' 5 was disabled and the channel was entered into Technical Specification Action Item Logbook '(#C1-95-01807). as inoperable. Problem Investigation Process (PIP) 1-C95-2245 has been initiated
" for' tracking purposes;- work request 95048483 has been initiated to perform corrective maintenance on this instrumentation. This work will be performed during Unit 1 Cycle 9.
Redundant monitoring of the upper vessel data collection region is afforded by LPMS Channel 6 (Upper Rx Vessel 240 Deg).
Since this channel has been inoperable for 30 days, this report is submitted pursuant Selected Licensee Commitment 16.7-4 and Technical Specification 6.9.2.
