Dear Mr. Ebneten Carolina Power & Light Company (CP&L) hereby requests the NRC to:

1.

Pursuant to 10 CFR 55.11, approve an exemption from the requirements of 10 CFR l

55.35(a) for an applicant.

l 2.

Pursuant to 10 CFR 55.35(b), excuse the applicant from re examination on the l

operating test which he has successfully completed.

The applicant was administered a Senior Reactor Operator (SRO) license examination during i

the week of October 23,1995 and successfully passed the operating test. However, the j

opplicent failed to pass the written portion of the examination.10 CPR 55.35(a) states that en opplicant whose application for a licensa has been denied because of failure to pass the j

written examination or the operating test, or both, may file a new application two months after the date of the denial. CP&L is requesting this exemption for the applicant to allow him to file a new license application, NRC Form 398, prior to the end of the specified two 1

months after the date of denist.

Also, CP&L is requesting that the applicant be re administered the written examination t

during the week of December 18,1995, which is a previously scheduled examination

. period.

No signifloant performance issues were identified in the NRC Upgrade Ucanse j

Operating Examination adminiatored on October 23 24,1995.

The applicant satisfactorily completed CP&L's written and operating examinations

. administered in accordance with our Upgrade Ucense Program.

i 4.

The applicant haa successfully completed CP&L's requirements to be licensed as a 1

Senior Reactor Operator, and has also sucessafully performed a minimum of five j

significant control manipulations which affected reactivity or power level.

l Operations and Training management at the Brunswick Plant have reviewed the appiloont's performance to date in the Upgrade Ucensing Program and believe that, with the prescribed i

remediation, the applicant will be ready for re-examination of the written Senior Reactor l

Operstar licenas examination in December 1g95.

I Attached are the new license application, NRC Form 398, for complying with 10 CFR 55.31 l

and 55.35, and a new certification of medical condition, NRC Form 396, for complying with j

10 CFR 55.21,55.23,55.27, and 55.33(a)(1).

l Purausnt to 10 CFR 2.790(a)(6), the attached information should be withheld from public disclosure.
