Dear Sir:

The NRC was notified of a potentially reportable deficiency in Reference A and an Interim Report was _ transmitted by Reference B. At that time, it was estimated that a Final Report would be available by August 17, 1984.

Due to the extensive investigation and evaluation required, a revised Interim Report is attached. It is now expected that this information will be finalized by September 18, 1984, at which time a complete report will be submitted.

INTERIM REPORT - DER 84-24 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1 I. Potential Problem There have.been several instances of Combustion Engineering (CE) supplied Limitorque valve operators loosening from their Borg Warner valve bodies after the bolts securing them had been torqued.and inspected. The following are the valves in question:

7) 1JSIAUV688 During .the preoperational testing of the "SI" system, actuator problems such as low phase current readings, torque switch settings and stalled motors due to locked rotors had been encountered with SI system valve motors. These instances are documented in NCR SE-3994 and NCR SE-3967.

Per the suggested disposition of these NCRs, the original motors were replaced with new actuators supplied by CE. The work was implemented by Startup Work Authorization (SWA) #19752 for Items 1 and 2 and by SWA 19554 for Items 3, 4, 5, and 6. Subsequent to the motor changeout, it was discovered that the valve operators were loose on all seven valve bodies. SWA 19861 was issued to torque the operator to valve- bolts on Items 1 and 2, and SWA 20030 was issued to torque the bolts on Item 7.

II. Approach To and Status Of Proposed Resolution In response to Bechtel's request for possible cause and safety -

implication of this deficiency, Combustion Engineering has provided in Letter No. V-CE-30486 the guidelines to follow when torquing the yoke to motor operator connection bolts. As specified in the valve instruction manual, the valve stem has to be in the correct position for proper fastener torquing.

Since it is indeterminate whether or not the valve stems were in the correct open position prior to torquing, NCR SM-4407 has been issued to retorque the bolts.

The interim disposition to NCR SM-4407 provides a resolution plan to the deficiency. The plan is to stroke each valve to full open, loosen the her bolts, re-torque the her bolts per vendor requirements, operate the valves several times and then check the bolt torque to ensure that the torque has not I changed. l
