Pursuant to Section 6.'9.'l of Appendix A, Technical Specifications to Facility Operating Licenses NpF-10 and NPF-15 for San Onofre Units 2 and 3, respectively, this submittal provides the required Special Report for inoperable accident monitoring instrumer,tation.

On August 4, 1984, at 0855, with Unit 2 in Mode 1 at 97% power and Unit 3 in Mode 5, Purge / Vent Stack Wide Range Monitor 2RE-7865 was removed from service for performance of the 92-day surveillance.

Flow rate estimates, grab samples and auxiliary sampling were initiated in accordance with Limiting Condition for Operation (LCO) 3.3.3.9, Action Statements 36, 37 and 40, respectively.

2RE-7865 was not restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> to permit corrective maintenance to be performed on a system valve, although the valve deficiency had not renderdd'the monitor inoperable. The preplanned alternate method of monitoring was initiated per LC0 3.3.'3.6, Action Statement 22.

Monitor 2RE-7865 was tested in accordance with Station Procedure S023-3.21.1 and returned to service at 2130 on August 9, 1984.

Neither the health and safety of plant personnel nor the public were affected by this event.
