t DUKE POWER COMPANY CATAWBA NUCLEAR STATION REPLY TO NOTICE OF VIOLATION 413, 414/95-16-01 Notice of Violation Technical Specification 6.8.1, Procedures and Programs, requires, in part, that written procedures be established, implemented, and maintained covering the activities referenced in Appendix A ofRegulaton) Guide 1.33, Revision 2, February 1978.
As referenced, this includes procedures for performing maintenance that can affect the performance of safety related equipment.
Implicit in this requirement is the stipulation that the procedures be adequatefor the circumstances.
Contrary to the above, on September 19,1995, procedure MP/0/A/7400/42, (Emergency Diesel Generator) Turbocharger Removal and Replacement, was inadequate in that provisions were not provided to install and maintain lock washers in the turbocharger mounting bolt anangementfor the Unit 1 and Unit 2 Emergency Diesel Generators. Lock washers were specified for use in this application on the vendor drawing for the turbochargu mounting brackets (drawing CNM 2301.00-023-001). Failure to install a suitable lock washcr configuration caused 3 of 4 mounting bolts tofail on the 2B Diesel Generator Right Bank Turbocharger and introduced a potential common modefailure into the Emergency Diesel Generators on both units.

RESPONSE
1.
Peason for Violation This violation is attributed to personnel error.
Engineering and Maintenance personnel failed to properly evaluate and document the deletion of the lock washers from the diesel generator turbocharger mounting bolt arrangement in a 1984 change.
Lock washers were originally included in the Urdt 1 and Unit 2 diesel generator's turbocharger mounting bolt arrangement. In 1984 an option for modifying the mounting bolt arrangement was implemented. This option allowed the bolt holes in the mounting bracket for the turbochargers to be elongated to facilitate Nrbocharger alignment. For te elongated bolt holes, a one-eighth ivh tainless steel plate was also installed under the mounting bolt heads to ensure that an adequate bearing surface was maintained to secure the bolts. It was during this time that the lock washers were deleted.
In September,1995 the problem with the turbocharger mounting arrangement was identified. In reviewing the records and information from the 1984 time frame it was determined that the deletion of the lock washers was a conscious decision but was not changed under the modification process and therefore not properly evaluated and documented.

Corrective Actions Taken and Results Achieved An engineering e. valuation of the Unit I and 2 diesel generator turbocharger mountics arrangement was performed. Based on the results of this evaluation, modification CNCE-7308 was initiated to properly mount the turbochargers. This modification was implemented on all diesel generator turbochargers on September 23,1995.
As part of the modification
- process, Maintenance Procedure MP/0/A/7400/42, Turbocharger Removal and Replacement, was reviewed for necessary changes and subsequently revised to reflect the turbocharger mounting arrangement in accordance with the modification.
The procedure now requires the use of both lock washers and flat washers for turbocharger installation.

from 60-65 ft-lbs to 75-ft-lbs to further ensure that the holddown bolts remain secure.

Corrective Action to be Taken to Avoid Future Violations The corrective steps identified in Section 2. above are adequate to prevent further violations.
