Dear Mr. Hunter:
This letter provides the final 10CFR50.55(e) report concerning Isolation Relay Cards manufactured by Beta Products, Inc. This matter was initially reported by Kansas Gas and Electric Company (KG&E) on April 26, 1984, and additional information was subsequently provided in the Reference.
During preoperational testing of the chemical and volume control system, an isolation relay card used to monitor control power for valve BG-HV-8160 failed. Failure of this card caused the control power fuses for the valve to blow, which resulted in a loss of control power.
A follow-up investigation revealed 35 additional isolation relay cards that had the potential for causing similar failures. All of these cards are part of the Engineered Safety Features Actuation System and were manufactured by Beta Products, Inc.
To resolve this problem, the 36 affected cards were modified so that i
