Dear Mr. Keppler:

This letter is'in response to your Notice of Violation dated June 12, 1984, Inspection Report Number 50-461/84-15.

Illinois Power Company's response to the item of noncompliance is as follows:

The Notice of Violation states in part:

Sargent and Lundy Specification, K-2884, through Amendment 8, March 12, 1984, Component Supports, requires impact testing for supports integral to Class 1, ASME Section III, Subsection NF, 1974, including Summer 1974 Addenda.

Contrary to the above, four lugs identified in drawing RH-12 and attached to a 12" pipe spool by weld numbers 15E, 15F, 15G and 15H, were not impact tested as required for integral attachments to the 12",

.844 Wall Residual Heat Pipe, Class 1, Line IRH04B-12", which was impact tested in accordance with Subsection NB, ASME,Section III.

Corrective Action Taken and the Results Achieved A review was performed of large bore piping isometric drawings showing'ASME,Section III, Class 1 pipe.

Nine (9)

. piping spools were identified, as having lugs attached.

Of the nine (9)-spools identified, four (4) were determined to have lugs (four (4) lugs per spool) which had not been e

impact tested.

The four (4) nonconforming spools will be resolved by Nonconformance Reports (NCRs) 18443, 18462, 18463, and 18609.

II.. Corrective Action to be Taken to Avoid Further Noncompliance

.Baldwin Associates has performed a review to identify all instances of potentially unqualified lug material on ASME,Section III, Class 1 pipe.

Further, the impact test requirements for lugs will be reviewed with Baldwin Associates Piping / Mechanical Resident Engineering, Quality Assurance Procurement Engineering, and the Senior Piping Superintendent.
