Daket No. STN 50-482 Subj: Final 10CFR50.55(e) Report - Turbine Driven Auxiliary Feedwater Pump Terminal Strip

Dear Mr. Hunter:

The purpose of this letter is to provide a final report pursuant to 10CFR50.55 (e) concerning determinated leads within a control panel for the Turbine Driven Auxiliary Feedwater (TDAFW) Pump at Wolf Creek Generating Station (WCGS). This matter was initially reported to Mr. Lawrence Martin of the Nuclear Regulatory Commission, Region IV, by Mr. H.K. Chernoff of Kansas Gas and Electric Company (KG&E) on September 20, 1984.

While performing a preoperational test (SU3-ALO2) of the Turbine Driven Auxiliary Feedwater Pump, a throttle valve failed to open when operated from local control panel FC-219.

Investigation of this matter revealed loose connections on a terminal strip upon which field run cables were terminated. This system, including panel FC-219, had operated properly during previous component testing.

A Construction Work Permit (CWP) was issued to reterminate the conductors found determinated and to tighten all other connections in panel FC-219.

All activities described in the CWP have been completed.

In addition, evaluations of this matter have concluded that neither a design deficiency nor programatic breakdown caused the inadvertent determination of the aforementioned conductors.
