The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that cime, it was estimated that a

determination of reportshility woald he made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached. It is now expected that this information will be finalized by August 20, 1984, at which time a complete report will he submitted.

4 INTERIM REPORT - DER 84-42 POTENTIAL REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 1, 2, 3 I. Potential Problem Deaign Change Package (DCP) 10M-SI-302 was issued to replace environmentally unqualified torque switches in motor operators on valves 1JSIA-UV-674 and 676 with qualified switches as part of NUREG-0588 implementation. This work was completed i4 September 1983 by Work Orders 23744 and 23749.

1 On October 16, 1983, Startup Work Authorization (SWA) 15370/NCR SE-3095 was issued to troubleshoot the motor operators.

The work 4,ccomplished under the SWA inadvertently removed the qualified torque switchen from these motor operators and replaced them with the (previously renoved) unqualified switchea. On Oc tober 20, 1983, SFR/NCR SE-3179-was issued to document the fact that the original unqualified torque switchen were in the motor operators on valven IJSIA-UV-674 and ~676.

II. Approach To and Status of Proposed Resolution -

There are two specific areas of concern resulting from this deficiency:

1. Placement of unqualified torque switches on valves SI 674 and 676 motors which are used for containment recirculation sump outboard isolation, may impair the safety function of those valves.

Combustion Engineering, the supplier of the valves has been notified and requested to provide a determination of safety implications by August 1,1984.

2. The apparent absence of procedural controla to preclude substitution of environmentally qualified parts with unqualified components may cause recurrence of this condition.

Bec.htel engineering is currently reviewing this problem with APS to ascertain the root cause of thin condition and proper corrective action.

III. Projected Completion of Corrective Action and Submittal of the Final Report The complate evaluation and final report are forecast to be completed by August 20, 1984, i
