Dear Sir:
The NRC was notified of a potentially _ reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.
Due to the extensive investigation and evaluation required, an Interim Report is attached.
It is now expected that this information will bc

Potential Problem
- Limitorque type SMB valve operator Instruction. & Maintenance Manual 1(Bu11etin SMB1-82) notes on page 12 that. Sun Oil Co.
50 EP ' type lubricant was the standard lubricant for operators with serial nunbers up to 295809. -It is further noted.on page 12 that Sun 50 EP (lithium lead base) cannot be mixed with '
.DuEing development 'of-the preventative maintenance program, a computer search was performed on-limitorque type SMB valve :
operators. A total of 50 such limitorque. valve operators were) identified as having serial numbers' below 295809, and were presumably initially lubricated with Sun 50 EP; A11-50 valve actuators were lubricated subsequent. to delivery to the -
jobsite with Nebula EP-1 lubricant, which is a calcium complex base lubricant. Ability of valves to function as required is indeterminate.
In addition, Sun 50 EP lubricant is not qualified for use inside containment. Ten of the 50 subject valves are located inside containment.
II.
Approach To and Status Of Proposed Resolution Bechtel Engineering is corresponding with Combustion.
Engineering (C-E) and Limitorque Corporation to determine reportability and the corrective action.
C-E has f orecast September 21, 1984 for their input and Limitorque has forecast
. September 28, 1984 for their input.
III.
Projected Completion of Corrective Action and Submittal of the Final Report The complete evaluation and final report are forecast to be completed by October 30, 1984 I
