Dear Sir:

The NRC was notified of a potentially reportable deficiency in Reference ( A), an Interim Report was transmitted by Reference (B), a Time Extension was requested by Ref erence (C), and a Final Report was transmitted by Ref erence (D).

The NRC was notified in Reference (E) that the corrective action specified in Ref erence (D) f ailed to correct the deficiency and the subject DER had been re-opened. A revised Interim Report was submitted in Ref erence (F) and Time Extensions were requested in Ref erence (G) and (H). At that time it was estimated that a Final Report would be available by September 21, 1984
