OL Cover Letter for Documents Sunporting Stokes Affidavit on Eddleman 65 (Defective Concrete)
Dear Administrative Judges:
Enclosed are copies for you and all parties of the selected sample of documents illustrating facts cited in the affidavit of Charles Stokes filed June 14 in this proceeding.
By arrangement, Charles Barth of NRC Staff will receive a copy of the entire set l
of CP&L pour packages reviewed by Stokes.
I also possess copies of the other 95% of the cour packages as copied for me by CP&L; I
if anyone wishes to exanine them, clease contact me at 919-286-3076.
This letter will briefly describe th enclosures and the cart (s) of the Stokes affidavit of 11 June 1984 which each relates to.
40E The first enclosure is from pour ICBXW290001 of 7/23/82,
- n-(Stokes affidavit pp 5-7, starting low on page 5).
This includes no Es the Ebasco concrete specification stokes refers to on his page 6 gog i
Q and the related concrete strength reports.
The next is fron nour LCBSL216002, Beginning with the Field Inspection Report sheet on " Accidental Burning of Waterproof 1
Membrane" which is the Water.stop referred to by Stokes on op 9 &l0.

The table continues on later pages.
These are the out-of-specification slumps in this pour as cited by Stokes on page 9.

The fourth enclosure includes the cover sheet of the i
ICBSL216-002 Concrete Placement Report.
It documents the hot weather, (1st page, " Anticipated Special Weather Precautions",

and reproduces pages indicating repair work not completed (not covered by Stokes) on the waterstop 7-27-78 and also the last page has the note on clearance between asbestos board and cadweld under 1/2 inch.
This clearance problem is mentioned by Stokes on p.9 and is the same one.he mentions on p.8 as appearing with the same M "never again" language (" Future clearance to be 1/2 in minimum) on several other pours in the early going, i.e. ICBSL216001,-OO4 and 005).
The fifth enclosure is the concrete strength information i
from ICBX# 308001 of 8-29-83, a single sheet. (Stokes, p.7) 1

Note the reference to 9% air in pretest under " remarks".
The sixth enclosure is the entire nackage for ICB SL P16001.
It documents the alump, waterstop and vibration problems (Stokes, pp 8c and also'the asbestos board problem, e.g. on 216004 sheet 1 of 4, i

This package is as it came to me from CP&L except reproduced on front-and-back of pages to save paper, to the best of my knowledge.
The seventh enclosure is the concrete repairs report on the large void inn pour ICBSL 216002 (Stokes, pp 9-10).

The eighth enclosure is from 'ponr KICr37M219001 (Stokes pp 3-4) including the strength data on" cylinders (lact' 6 age of enclosure).
Note the approvals on the next~ to-last sheit iil spite of the problems noted by Stokes.
The comments on exposed aggregate are on the front sheet'uridor " comment N Clarification to Proposed Methoda", uppar right quad int of the sheet.

- This is intended to cover only those new design mixes, created by modification of deter =ined design mixes, which have not been proven by the test lab prior,to their R7 t
placement. When 100 or more test cylinders for a new design mix has proven the mix to be acceptable to the Engineer, the number of test cylinders r& quired for each 100 cu yd or fraction thereof may Le reduced to a minimum of 4.
The extra cylinders will only be tested if it is necessary to substantiate 7 or 28 day test results.
.5 Evaluation of Strength Tests ne concrete cylinders shall be tested for conpressive strength in accordance with ASTM C39. The strength level of the concrete shall be considered satis-factory if:
a - No individual strength test results falls more than 500 psi below the required class strength at 28 days.

Each 28 day strength test result shall be the average of two cylinders from the same sanple. De variation between the two cylinders shall be not more than five (5) percent of their average. A greater variation shall require testing R4 of the third (spare) cylinder to determine the average strength. If the third R9 cylinder strength " variation"is also greater than five (5) percent from the average, the R4 Owner shall determine the reason for such a wide variation in test results and rectify it.
