12 REQUIREMENT BQ-TOP-1, Section 7.2, Supplier Inspection, states:

No nonconformance exists. Material supplied was the stipulated brand from the stipulated manufacturer.

(Specifications C-231, Paragraph 17.1, C-195, Paragraph 5.1h and C-194, Paragraph 10.5.6.

3. ~ Reference Telex No. N103 dated April 18, 1984' from Larsen Products Corporation stating their. shelf-life position for welderete (attached).

Present manufacturer's instructions require welderete to be stirred prior to its use and stirring is verified by inspection using PQCI's C-1.10, Section 2.1 A.3, UP-C-1.012, Section 2.1 B3 and UP-C-2.008 Section 2.1 A.

Welderete is a trade name product manufactured by Larsen Products Corp.

This product has been used successfully for over seven years at the Midland Energy Center for Q-related application. The engineering requirements in Specification C-231, Paragraph 17.1, C-195, Para-graph 5.1h and C-194, Paragraph 10.5.6 require welderete by Larsen Products Corp. This requirement is also specified in the' Purchase Orders and is verified at receipt inspection. A listing of all purchase orders for welderete is available from Purchasing.

Block-29:.CAUSE No documentation of supplier evaluation and clarification was required for C of C approval.

WELDCRETE is a standard catalog iten manufactured by Larsen Products Corporation. The material, in accordance with 1982 Sweet's catalog Section 3.1/Lar, complies to the requirements of ASTM C-39,' C-78 and C-190 It is a standard product commonly used in the industry as a bonding agent.

If the material can be certified that it meets the requirements of ASTN C-39, C-78 and l

C-190, it can be used in safety, related structures.

Project engineering specified Codes 1 and 4 for procurement of VELDCRETE against Specificati~on 7220-G-33(Q).

Code 1 " certificate of conformance (C of C) to procurement document" requires supplier to submit a statement certifying that the material meets the requirements of applicable codes and standards.

3) Shelf life controls.

4) Alternative methods of assurance of acceptability for Q-related application.

II. BPCo Procedure FPC-8.000, Field Material Requisitions, Rav 8, cross references-SPCo Specification 7220-C-33Q - Q-listed Bulk / Shelf Bulk and ANSI N45-2.13 Quality assurance requirements for control of procurement items and services for

uclear Power Plants. Section 10 outlines the requirements for Certificate of Conformance, which was required by BPCo Purchase Order.

Contrary to the above, the Certificate of Conformance for Walderate did n6t provide fsr the requirensets of ANSI N45.2.13. Sect 10, Para 2.d. e, f.

The certificate should be attested to by a person who is responsible for this h

quality assurance function and whose function and position are described in the Purchaser's or Supplier's quality assurance program.

The certification system, including the procedures to be follo'wed in filling out a certificate and the administrative procedures for review and approval of the certificaces, should be described in the Purchaser's or Supplier's quality assurance program.
