Attachment Based on the results of an NRC inspection conducted during the period of March 1-31, 1984, and in accordance with the NRC Enforcement Policy (10 CFR Part 2,

. Appendix C), 47 FR 9987, dated March 9,1982, the following violation was iden-ti fied:

Failure to Follow Procedures Technical Speci fication 5.8.1 requires that " written procedures... be established, implemented, and maintained that meet or exceed the minimum requirements of Section 5.1 and 5.3 of ANSI N18.7-1972, and Appendix A of USNRC Regulatory Guide 1.33..."

Standing Order G-7 establishes the Operating Manual and states in Section 1.3 that " adherence to the Operating Manual is mandatory."

Procedure CMP-4.5, "Contairinent Purge Release Pennit," contains the in-structions for initiating and conducting a containment purge release.

Section VIII.6(c) states that, "during the use of the contairinent purge system, the release must be terminated if the X/Q value becomes greater than E-Sec/M3 (the limiting X/Q) based on C FM. "

Contrary to the above, on March 6,1984, during the conduct of Contain-ment Purge 84013, the licensee failed to tenninate the cnntainment purge when the 8:00 a.m. X/Q reading was greater than the limiting X/Q for the established flow rate.

This is a Severity Level IV Violation. (Supplement I.D.3) (285/8407-01).

Response

The corrective steps which have been taken and the results achieved.

The incident was discussed with the Operations personnel involved. A memo has been issued to the Operations Department outlining this incident and reiterating the appropriate requirements of a containment purge per-mit. A Procedure Change has been submitted to revise the Operators Check-list for the X/Q Log for Containment Purge. This change entails addition of a Shift Supervisor review of the required periodic data that is taken during a purge. The procedural change is being made to enhance the exist-ing procedure and will be completed by June 11, 1984.

To date, there have been no further violations of a containment purge pennit limiting X/Q.

The measures previously taken, as described in (1) above, are considered appropriate to prevent recurrence of. the violation.

The District is currently in full compliance.
