NPPD Response to IE Inspection Report No. 50-298/84-04
Dear Mr. Johnson:
This letter is written in response to your letter dated April 25, 1984 trans-mitting Inspection Report No. 50-298/84-04. Therein you indicated that one of our activities was in violation of NRC requirements.
Following is the statement of the violation and our response in accordance with 10 CFR 2.201:
Statement of Violation Failure to Accurately Perform a Part of Technical Specification Required Surveillance Test 6.2.4.1 Cooper Nuc1 car Station Technical Specifications, Section 4.6.3.1, states, "Whenever there is recirculation flow with the reactor in the startup or run modes, jet pump operability shall be checked daily by verifying that the following conditions do not occur simultaneously: a) the recircu-lation pump flow differs by more than 15% from the established speed flow charat6 2ristics, b) the indicated value of core flow rate varies from the value derived from loop flow measurements by more than 10%, c) the diffuser to lower plenum differential pressure reading on an individual jet pump varies from the mean of all jet pump differential pressures by more than 10%."
Contrary to the above, on February 28, 1984, the licensee failed to accu-rately perform Surveillance Procedure 6.2.4.1, Attachment A, page 6 of 15, Check 1.
The data entered in the blanks provided on Check 1 sere incorrect and the subsequent interpretation of that data would have indicated that the requirement of the Technical Specification, Section 4.6.E.1.C. quoted above, was not met.
The licensee recorded that Check 1 did meet the Technical Specification requirement.
This is a Severity Level IV Violation.

Mr. Eric Johnson M y 18, 1984 Page 2 Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
Corrective Steps Which Have Been Taken and the Results Achieved The Operations Supervisor discussed the incorrect entry of data and the lack of adequate supervisory review which could have detected that data was in-advertently logged in the incorrect blanks with the operator and supervisor involved. The operator and supervisor involved better understand the necessi-ty to be precise in performance of the daily jet pump operability check.
Additionally, during regularly scheduled weekly crew meetings with the Operations Supervisor, emphasis has been given to correct logging, inter-pretation of data, and thorough supervisory review of logs. The probability of recurrence of such an error has thereby been reduced.
Subsequent to the violation, the position of Control Room Supervisor (CRS) has been established.
One of the functions of the CRS (SRO qualified) is to review logs prior to Shif t Supervisor review. Thus the CRS provides an on-shift double check of data lodged by the reactor operators. It is expected that this additional review will prevent violations of this type. The position of the CRS was not created in response to this violation; however, it is believed the CRS review has already reduced errors of this type.
Corrective Steps Which Will Be Taken to Avoid Further Violations The daily jet pump operability check surveillance log is being revised.
Investigation of why the data was entered in incorrect blanks revealed the log format was unnecessarily confusing. The new format reduces redundant data logging and simplifies supervisory review.
The Date When Full Compliance Will Be Achieved Cooper Nuclear Station is currently in full compliance with the requirements.
