QAR's numbers RC-00491 and RC-00492 have been issued to track the status of sctions to resolve the items reported on the subject NIR's.

Paga 1 of 1 NIR 029 ATTACHMENT 1 The document referred to'in NIR 029 as a memorandum was actually a list of handwritten notes concerning what appeared to be possible clerical errors or omissions in procurement documents. It was compiled by the APFQCE during a cursory preliminary review of Material Receiving Reports and related documents in preparation for initiating the actual receiving inspection process of the purchased items. This handwritten list was.provided to Purchasing with a verbal request that they investigate and-clarify or correct the listed. items, as necessary. All listed items have been, or are now in the process of being corrected. The APFQCE has been instructed that in the future any similar notes and requests for clarification or action are to be documented on a Request for Information form, in accordance with MPQAD Procedure M-17.

Any nonconforming conditions identified during the actual receiving inspection have been, and will continue to be documented on a Nonconformance Report, in accordance with MPQAD Procedure F-2M.

l Case No. 1 QC agrees the conditions existed as described and that they were contrary to the requirements of the indicated PQCI. These errors were corrected by QC upon receipt of verbal identification of the problem from S&W prior to the issue of NIR 031.

Case No. 2 QC agrees.the conditions existed as described and that they were contrary to the requirements of the indicated PQCI. These errors were corrected by QC upon receipt of verbal identification of the problems from S&W prior to the issue of NIR 031.

Case No. 3 (1) IR No.s VC-1.40-2, VC-1.40-3, and VC-1.40-5.

Visual examination clearly indicates the vertical steel plates noted on the NIR were in place at the time of concrete placement in 1977 and have not been removed since that time. Paragraph 4.3.1 of the CCP states: "The verification program is the activity undertaken to establish, using a variety of methods, that the hardware items completed and inspected prior to December 2, 1982 have an acceptable quality status and that prior inspections were performed in an acceptable manner." Since the areas behind these a plates were not and could not have been the subject of an inspection prior to December 2, 1982, these areas are not considered inaccessible and included in the QVP.

(2) IR No. VC-1.40-8.

QC agrees the conditions existed as described. A DIIA has been initiated for activities 2.3 and 3.2 and this j fact noted in the appropriate blocks of the QC IR.

(3) IR No. VC-1.40-10.

.-Item 1 - A DIIA was initiated for activity 3.2 on April 2, 1984, however it was not listed as DIIA in block 14 of the QCIR. This error has been corrected.

- Item 2 - A generic DIIA covers this activity and it should have been so noted on the QCIR. This error has been corrected.

- Item 3 - DIIAs have been initiated for the areas listed on the NIR which were covered by attached plates and the QCIR annotated accordingly. It should be noted that, due to the small areas covered by these plates (6" x 3" x "),

QC had originally considered an inspection of the area surrounding the plates to be an adequate indicator of the conditions actually under those plates and, accordingly, no DIIAs had been written prior to receipt of the NIR.

This matter has been referred to MPQAD Plant Assurance Division for their consideration. Pending any determination to the contrary, however, QC will treat all areas behind attachments, no matter their size, as inaccessible.

(4) IR No. VC-1.40-29.

A DIIA was written for activity 2.3 and should have been referenced on the QC1R for activity 3.2, also. This error has been corrected.

(5) IR No. VC-1.40-30.

DIIAs have been initiated.for the areas listed on the NIR which were covered-by attached plates and the QCIR annotated accordingly. It should be noted that, due to the small areas covered by these plates (6" x'3" x "), QC had originally considered an inspection of the area surrounding the plates to be an adequate indicator of the conditions actually under those plates and, accordingly,'no DIIAs had been written prior to receipt of the NIR. This matter has been j referred to MPQAD Plant Assurance Division for their

consideration. Pending any determination to the contrary, I ' however, QC will treat'all areas behind' attachments, no l matter their size, as inaccessible.

(6) IR No. VC-1.40-33.

The plates listed on the NIR for this item are in a t'urned over area and were installed after QVP was completed.

The areas under these p'lates were accessible and were in-spected as part of the QVP.

The conditions descrioed in the NIR for these items are a duplication of the problems identified under Case No. 3, above, and have been addressed by QC's response to Case No. 3.

(2) IR No. VC-1.40-9.

QC agrees DIIA should have been entered on the QCIR block 14 for activity 2.3. This error has been corrected.

Case No. 5 QC agrees the conditions existed as described and that they were contrary to the requirements of the indicated PQCI. These errors were corrected by QC upon receipt of verbal notification of the problems from S&W prior to the issue of NIR 031.

Case No. 6 (1) IR No. VC-1.40-2; item 1.

QC agrees this condition existed as described and that ,,

it was missed. NCR C-3565-V was initiated to track th'is item upon receipt of verbal notification of the problem from S&W prior to the issue of NIR 031.

(2) IR No. VC-1.40-2; item 2.

QC agrees this condition existed as described and that it was missed. NCR-3565-V was initiated to track this item upon receipt of verbal notifica' tion of the problem from S&W prior to the issue of NIR 031.

(3) IR No. VC-1.40-10.

Upon reinspection by QC, some exposed tie wires were observed; however, no rock pockets, as described in the NIR, were noted. Since exposed tie wire is considered cosmetic only, and not detrimental to the structural integrity of the concrete, QC has taken no further action on this item.

Case No. 9 QC agrees that the locations given on the indicated irs was in insufficient detail to find the field location of the particular pour. This condition was corrected by QC upon receipt of verbal identification of the problems from S&W prior to the issue of NIR 031.

Case No. 10 QC agrees the conditions existed as described. These errors were corrected by QC upon receipt of verbal identification of the problems from S&W prior to the issue of NIR 031.
