Beta Products Inc. Isolation Relay Cards
Dear tir. Johnson:
This letter provides an interim report on a potential 10CFR50.55(e) concerning isolation relay cards manufactured by Beta Products Inc.
This matter was initially reported by Mr. H.K. Chernoff of Kansas Gas and Electric Company (KG&E) to Mr. John Boardman of the Nuclear Regulatory Commission, Region IV, on April 26, 1984.
During preoperational testing of the chemical and volume control (BG) system the isolation relay card used to monitor 125 volt DC centrol power for a valve (BG-HV-8160) failed. Failure of this relay card caused the control power fuses for this valve to blow, resulting in a loss of control power.
Initial investigation indicates that other isolation relay cards manufactured by Beta Products Inc. may fail in a similar fashion. These relay cards are a part of the engineered safety features actuation system (ESFAS). A KG&E Startup Field Report has been written to document and resolve the existing condition.
The status of this potential 10CFR50.55(e) will be carried on the monthly status update as file 53564-K136. The final report cr any significant new information will be submitted by separate correspondence.
