DUKE POWER COMPANY McCU1RE NUCLEAR STATION RESPONSE TO VIOLATION 50-370/84-04-01 Viointion 50-370/84-04-01. Severity T.evel IV:
Technten1 Specifications 6.8.1 requirca current written approved proceduren be established, implemented and maintained covering startup of safety reinted equipment, annunciator /ninrm renponna and surveillance testing of safety reinted equipment.
Station Directivo 3.1.4 requirca the oporntor to monitor and acknowledge instru-mentation displays and alarms, and to initiate prompt correctivo action on receipt of irregular operating conditions.
Contrary to the above, approved procedures and Station Directivo 3.1.4 woro not followed in that the following conditions wore noted:
1.
On January 15. operations personnot fatted to follow proceduru op/2/A/6200/01, Chemical and Voluma Control System, by not verifying suction hender valvo to be open prior to pump start. This resulted in the destruction of the 2-11 contrifugal charging pump.
2.
On January 15, operations pornonnel failed to abido by the requirements of Station Directivo 3.1.4. Conduct of Oporationa, in that during n 12 m8nuto parlod immediately preceding the destruction of tho 2-B contrifugni charging pump, 18 high VCT pronsure and/or high VCT icvol alarms woro received without prompt correctivo action.

On February 2 nn cloctronten technician failed to follow Stcp 10.6.2 of Proceduro IP/0/A/3010/05 by erroneounty tripping the main Train B reactor trip bronker instond of the bypann bronkar. This renuited in a unit trip from 89%
power.

Duko Power Company agreen that McGuiro Nuclear Station pornonnel did not follow the Chemient and Volumo Control Syntem (NV) opornting procedure when valvo positions woro not verified prior to starting NV pump 2A.
During operation of NV pump 2A, pornonnot did not immediately identify the loan of nuction to the pump, denpito control board indientionn and indirect Operntor Aid Computer Alarmn. Thono events renulted in pump damngo. An explanation of the circumstancon tending up to. the event and correctivo actions taken nro doncribed in Licenneo Event Reports 50-370/84-02 and 50-370-84-04.
In n noparate incident, a McGuiro Nucionr Station electronics technician did not proporly follow the portodic tent proceduro when ho inndvertently open tho Train 11 renetor trip hronker. Thin occured during performance of the SSPS Periodit: Tent abovo Ronetor Coolant System Prennuro of 1955 pai and renutted in n Unit 2 ronctor trip. 1.icennco Event Report 50-370/84-05 providen nn explanation of the circum-stancon loading up to thin event and the corroetivo actionn taken.
In addition to the correctivo netions doncribed in the 1.ER ndditional bronker Inbots have boon

placed on the interior of breaker compartments to assist in properly identifying breakers.
In order to make other personnel aware of incidents caused by personnel error, McGuire Nuclear Station Incident Investigation Reports involving personnel error are reviewed during all crew meetings. McGuire is presently in full compliance.
