AUDIT FINDING REPORT FINDINGS s

AMS-84-26-3F - Corrective Action Response, JTChristy Item 1: QC personnel will be reminded of the existing requirements in E-3M, Rev 2 Attachment 7.5 as modified by DR 208.

To be completed by June 15, 1984.

Item 2: Organizational responsibility for Corrective Action for this item shculd be MPQAD - PAE, GEParker instead of JClark.

AMS-84-26-4F - Corrective Action Response, DEBeaudoin Item 1: Individuals who have not received N-7 training will be identified.

The individuals will complete training by June 15, 1984.

Item 2:

Procedure B-2M-1 Rev 4 will insure that individuals are scheduled for required training.

Supervisors will submit Initial Training Request forms to the Training Schedulers.

Therefore, those individuals requiring N-7 training will be icentified as they start their assignments.

This new revision of B-2M-1 became effective on April 30, 1984.

QVP has been revised and is in the review / signature cycle.

Corrective Action will be completed by June 15, 1984.

UNRESOLVED ITEMS AMS-84-26-2U - Corrective Action Response, JTChristy Item 1:

Rev 0 VIRs.were not voided because the inspections documented on them were valid records. We do not consider this in violation of PSP G-6.1 and therefore do not consider this an Audit Finding.

Item 2:

QC is evaluating the method of transferring inspections completed on superseded PQCIs to later revisions of the VIR.

QC personnel will be informed of the proper method for referencing inspections on revised inspection records.

AMS-84-26-3U - Corrective Action Response, DATaggart Item 1:

The process,to handle inaccessibility during Status Assessment was established to be different from QVP at the outset, and is not construed to be a quality problem. 'However, we have evaluated this established difference and determined that from an efficiency and.

consistency standpoint we will adjust the process of handling inac-cessibility during Status Assessment. MPQAD will take the following action:

Revise MPQAD procedures to require during Status Assessment; a DIIA be generated in lieu of a circled letter in the acceptance column, if the attribute has been previously inspected.

The DIIA would then be processed by the Verification Programs Management Section as in QVP.

If the item had not been previously inspected, an NCR (or generic NCR for a generic condition) would be documented.

For consistency purposes, MPQAD will retroactively generate DIIAs for those, inaccessible Status Assessment items which are currently documented by suffix "a".

A date for the completion of the procedure revision and the retrofit change is currently under evaluation. MPQAD will provide an updated response by June 15, 1984.

AMS-84-26 4U - Corrective Action Response, DEBeaudoin Item 1, 2:

Specific organization N-Series training requirements will be docu-mented in a new revision of the MPQAD Training Plan which is scheduled to be issued on July 1, 1984 which will complete this Corrective Action.

The Training Plan will become MPQAD's governing document for training requirements.

AMS-84-26 Corrective Action Response, LRHowell Item 1:

Proposed Corrective Action - Procedure M-15 will be changed to clearly state that QARs will be prepared for all programmatic deficiencies.

QVP has been revised and is in the review / signature cycle and MPQAD Procedure N-5 is to be deleted.

Corrective Action will be completed by June 15, 1984.

AMS-84'-26-10U - Corrective Action Response, DATaggart Item 1, 2, 3:

The procedures affecting CCP activities with identified Audit Findings were addressed separately.

The Program Development Group has recently been transferred to the Plant Assurance Programs Branch.

Currently we have initiated an internal evaluation of the MPQAD Procedural process with regard to timely revision processing.

This evaluation is currently being scoped and is expected to start on May 22, 1984.

MPQAD will* provide an updated response by June 15, 1984.

AMS-84-26-llU - Corrective Action Response, JTChristy Item 2:

Due to space limitations TQRs have not been assigned desks in the MPQAD area; however, all correspondence is delivered to TQRs in the MPQAD area and TQRs are permanently assigned and report to MPQAD QC areas.. We consider this as a resolved item at this time.
