Interim 10CFR50.55(e) Report; Fisher Controls Valve Operator Bracket Welds

Dear Sir:

On April 19, 1984, we reported a potential 10CFR50.55(e) deficiency to Region 1 [ Reference (b)] regarding weld deficiencies on Fisher Controls Company valve operator brackets.

It has been determined that this item is reportable under 10CFR50.55(e).

The following information is being filed pursuant to the interim reporting provision of 10CFR50.55(e)(3).

A. Description of Deficiency Numerous weld deficiencies were discovered on valve operator brackets for service water system valves (ASME III & ANSI B31.1) manufactured by Fisher Controls Company. The welds do not comply l with the specification requirements of the American Welding Society (AWS) standards in regard to undercut, size, voids, fusion, and corner fill.

The brackets are mounted on butterfly valves for the service water system only. To date, seventy-two brackets have been determined to have weld deficiencies.

l B. Analysis of Safety Implication Failure of the bracket welds could affect the operability of the valves.

1. All Fisher Controls valve operator brackets are being inspected for weld deficiencies. Those brackets having weld deficiencies will be returned to Fisher Controls for repair. Repaired brackets will be inspected upon receipt.
2. United Engineers has instructed Fisher Controls on February 24, 1984 to reinstruct personnel on welding acceptance criteria.
3. A review of purchasing documents will be performed to identify any other Fisher Control product used on the Seabrook Project.

We estimate that all affected valve brackets will be repaired by June 1, 1984. Region 1 will be notified upon verification of completion of the corrective action.
