Significant Deficiency R; port No. SD 413-414/83-16 Final Report The Deficiency Report dated November 23, 1983, identified 8,500 mechanical snubbers as potentially having defective capstan springs.
ITT Grinnell records indicate that of the 8,500 mechanical snubbers, 187 were shipped to Catawba Nuclear Station.
Of the 187, 109 were Size 1 snubbers and 78 were Size 3 snubbers.
We have reviewed our receiving records and confirmed that the snubbers in question were received at Catawba.
A complete search of the site has been conducted to locate the snubbers in question.
A review of transfer requisitions revealed none of the subject snubbers were transferred to other Duke sites.
It was also verified that none of the subject snubbers are currently with the vendor for repair.
The results of the search are as follows:
Of the 109 Size 1 snubbers, 99 were located leaving 10 unaccounted for.
Of the 78 Size 3 snubbers, all have been accounted for.
The following serial numbers are those of the Size I snubbers which were not located:
16761 20486 16815 22450 20441 22494 20456 22497 20477 22503 The search included the warehouse and all installed supports that utilize Size 1 and 3 mechanical snubbers.
The subject snubbers that were found installed have been removed and replaced.
All of the subject snubbers, that were located, are stored in the warehouse NCI area awaiting repair 1
arrangements to be finalized.
Therefore, none of the subject snubbers are installed.
Since all of the subject snubbers could not be located, we have developed a program to prevent the installation of any subject snubbers.
The appro-priate construction procedure has been revised to list all the serial numbers of the remaining sabject snubbers.
This list will be checked by the craf tsman when he records the serial number, of Size 1 snubbers, on the appropriate process control.
The appropriate M-51E (Support Inspection Instructions) has also been revised to make it a requirement of the inspec-tor to review this list when he/she verifies serial numbers during mechani-cal inspection.
The QA-301A Form (Final Review of Hanger Fackage) has been revised to make it a requirement to verify that all serialized compo_-
nents have a data report.
A copy of NCI 17249, which documents this prob-lem, has been attached to each affected data report.
When QA reviews each data report, they will at that time be able to determine if it is a serial number for one of the subject snubbers.
Full compliance has been achieved.
