FONTIrFT f n NUCLEAR GDDATIM) pIR.T Docket No. 50-263 License No. DPR-22 On February 3,1974, during routine transfer of torus water to radaste, the "A" Residual Heat Removal (RHR) toms cooling injection valve operator (bO-2008)
- otor failed due to stalled rotor current overheating the windings. The rotor stalled because the valve stem travel limit switches became nisaligrad. The misalignment was caused by the stem follower failing to retain the stem in its proper position, the stem position in relation to the stem followr has a direct relationship to the alignment of the stem travel limit switches.
Further investigation and repair are in progress. The valve can be operated manually if required. This failure kloes not affect LPCI operability.
Isolation features are preserved by the upstream isolation valve.
