We, the welding inspectors, felt like we had lack of support from 14 QA management, and some QA Procedure's were not being followed.

I talked to several people in the Charlotte offices, but I don't 20 remember their names.

I wrote some of my concerns on a document which is attached to 26 this. testimony '(Attachment A).

I turned this document in to the 27 QA ' office.

However, since I submitted this document I have _had p.

1 other concerns relating to harassment and welds in the plants.

2 These concerns are described in responses to later questions.

DID YOU EXPRESS ALL OF YOUR CONCERNS?

6 A.

No. I have additional concerns related to harassment and welds in 7

the plant.

With regard to harassment three incidents have 8

' occurred.

The first is while running M4-I's,

I had some 9

harassment from other inspectors.

Some times, inspectors under my 10 control found welds that did not meet the L-80 procedure and they 11 were - NCled.

Other inspectors who had previously approved the 12 welds, gave me a hard time for turning this in.

I did not express 13 this because, I felt like I was just doing my job.

14 The second incident related to harassment involved another J

15 inspector, Max Reep, who was harassed by craft in hio.ctempt to 16 NCI some weld filler material.

17 A third incident relating to harassment involved myself. While 18 running M4-I's a few months ago, we found some welds that had a 19 little more reenforcement than was allowed by L-80 Construction 20 Procedure.

These were signed Loff by a Weld Inspector that had 21 come from McGuire.

I mentioned the welds to his foreman, as I had 22 been instructed to do.

This Welding Inspector chewed me out and 23 said I was up to my old tricks again. -He also made the statement, 24 I'll get your f face off the M4-I job.

I went to file 25 harassment charger. on him because I had had all I could stand.

26 The charges were smoothed over and one week later, I was removed 27 from the M4-I job.

I'm not harassed anymore, but I don't feel like U~

28 this was handled in the right way.

3' the M4-l's I had rerun and he said that I shouldn't be keeping 4

notes.

5 Despite the harassment, I feel that the inspectors did not let 6

it affect them doing their job.

7 With regard to concerns about welds in the plant, my concerns 8

stem from four (4) incidents.

First, it was recently discovered i

9 that a welder had made a large amount of bad welds on a Class C 10 system, they just finished repairing the last one a couple of weeks 11 ago.

It makes me wonder if there are other welds made that are 12 unsafe. We, inspectors, could only check the outside.

13 Second,- I also saw a welder that had his stainless pg 14 certification, and he should not have-been certified, he was having 15 a lot of trouble on his root passes.

16 Third, I ran a lot of M4-I's.

I got to look at a lot of welds in 17 all areas.

I did find some that didn't have enough weld metal and 18 some that had too much.

I looked at a lot more'of good welds than 19 bad ones.

Some pipes had to be cut out 'due. to excessive pitting 20.

or other vendor defects.

This was a small amount due to the 21 amount of piping I checked.

I believe that what I checked and 22 later had fixed met what our QA Program called for.

- Fourth, 'some welds on the system.where we only checked 24 finals may not be made to a Construction Procedure and they may

(1) A few months ago, a Charlotte Observer reporter called me at 8

home by phone and wanted information for an article in the 9

paper.

10 (2) A few months ago, a lady from New York called and asked if I 11 felt like the plant was constructed safely.

PLEASE SUMMARIZE AND EXPLAIN ~ WHAT YOU WERE TRYING TO 13 COMMUNICATE BY YOUR CONCERNS.

14 A.

I was trying to point out that we had harassment from Construction 15 personnel and there were some things we had no control over, like 16 uncontrolled weld filler material.

I was also concerned that some 17 fits were being made that violated Construction Procedures.

WERE YOUR CONCERNS INVESTIGATED BY THE TASK FORCES?

19 A.

Yes.

I attended a meeting with the task force and we discussed my 25 concerns.

It was brought out that some of my concerns were on 26 piping systems that didn't carry any hot material.

Max Reep, had some trouble about some uncontrolled welding 10 material recently, I don't think the changes helped this situation 11 any.

THE WELDING INSPECTOR CONCERNS HAVE BEEN 13 CHARACTERIZED AS CONCERNS ABOUT THE QUALITY AND 14 SAFETY OF CONSTRUCTION AT CATAWBA.

DO YOU AGREE OR 15 DISAGREE WITH THAT CHARACTERIZATION?

16 A.

Agree.

Welding Inspectors are to make sure all welds they check 17 are sound welds and that the Construction Procedures are followed 18 to insure that the plant is safe.

However, systems has shown me 19 figures where they only need so much pipe _ thickness in a system 20 and our pipes were thicker than what was needed, so it should be 21 safe as far as pipe thickness.

DID THE EXPRESSION OF YOUR CONCERNS INDICATE YOUR 23 BELIEF THAT THERE WAS A BREAKDOWN IN THE QA PROGRAM 24 OR INDICATE THAT THE QA PROGRAM' WAS NO LONGER 25 WORKING? - EXPLAIN.

26-A, The QA Program was working I guess, but maybe not the way the

We should have had more 28 control over things we found on random inspection.
