Dear Mr. Gagliardo:

This latter provides the final 10CFR50.55(e) Report concerning missing wires on the Load Shedder Emergency Ioad Sequencer (LSELS). This matter was initially reported by Mr. Harold Chernoff of Kansas Gas and Electric Company (KG&E) to Mr. William Johnson of the Nuclear Regulatory Commission, Region IV, on February 27, 1984.

During component testing it was discovered that vendor wires werc missing from tw logic rack assemblies (NF173-1, NF173-2) of the LSELS. The subjcct wires are located between sequencer "A" (Slot #11, Pin #90) and the relay driver (Slot #17, Pin #86). These wires should have been installed during fabrication by the vendor, Consolidated Controls. The missing wires could possibly have prevented relays which deenergize the feeder bus from properly actuating on an under-voltage signal.

Once this condition was identified, Consolidated Controls was contacted in order to facilitate resolution of this matter. Subsequently vendor representatires installed the two missing wires in the logic rack assemblies. This work was controlled and documented by a Construction Work Permit (CWP). As a result of the aforementioned activities, KG&E deems that no further corrective action is necessary.
