, RESPONSE TO VIOLATIONS 50-369/83-47-01 AND 50-369/83-47-02 Violation 50-369/83-47-01, Severity Level IV:

l Technical Specification 6.8.1 requires that current written approved procedures be used to facilitate and conduct of surveillance testing and maintenance that can affect the performance of safety related equipment.

Technical Specification 3.3.1, Item 6.c. requires one operable source range detector channel during operation in Modes 3, 4, and 5.

Contrary to the above, on November 17, 1983, during the performance of procedure 1P/0/A/3100/06, a' functional test of source range instrumentation, the technicians failed to follow the procedure, which resulted in a loss of both source range channels.

Response

Duke Power Company agrees that a. violation occured at McGuire Nuclear Station when a procedure for_the source range.instrur.entation functional test was not adequately followed by technicians. This event was initially reported to the NRC in Licensee Event Report R0-369/83-lll. It has also been determined that this procedure con-tained an incorrect step. The procedure has been rewritten to make corrections and 4

to include independent verification sign-off steps.

Instrumentation technicians have been scheduled to attend a formal vendor system training class in order to obtain a better understanding of the Solid State Protection System. These corrective measures'will assist in preventing further violations in this area. ' McGuire Nuclear Station is perently in full compliance.

Violation 50-369/83-47-02, Severity Level IV:

Technical Specification 6.8.1 requires that current written approved procedures be used to facilitate the conduct of' surveillance testing and maintenance that can affect the performance of safety related equipment.

. Technical Specification 3.6.1.3 requires that.each containment air lock be operable with both doors closed except when the air lock is.being used for normal transit entry and exits through the containment, then at least one air lock; door.

shall be closed wh'en in Modes 1, 2, 3. and'4. -The Action Statement of this specification with one containment air lock door inoperable requires that the operable airflock door be maintained closed.

Contrary to.the above, on November 11, 1983, two Instrument and Electrical

. technicians while' performing maintenance on Unit 1 personnel air-lock with-the outer door open,-failed to use a procedure, resulting in a loss of containment integrity.when they inadvertently deflated the seals offthel inner door. There-after,~the operable. door.was immediately secured.

' Response:

Duke Power Company agrees that a violation occured at McGuire Nuclear Station

- when technicians failed to use procedure for maintenance of the personnel air lock, resulting'in a temporary loss of containment integrity. The procedure available was not adequate in that it did not contain sufficient guidance and prec'autions for the work being performed. A change-has been made to the pro-cedure as described in Licensee Event Report R0-369/83-107.

the -inner and outer doors. This interlock will prevent the door seals from

% fication will prevent further violations in this area. McGuire Nuclear Station is presently.in full compliance,
