The following does not apply to information provided to Stone and Webster Soils Assessment Team When S&W-CIO requests documentation from MPQAD personnel action shall be taken as follows:

1.

Documentation Supplied At The Time of Request The MPQAD individual supplying the documentation shall:

a.

Complete the Stone and Webster - Construction Implementation Overview Documentation Transmittal (Attachment A);

b.

Obtain the+ signature of the appropriate Section Head or higher; c.

Forward the original of the transmittal to S&W-CIO without another copy of the documentation unless saquested other-wise by S&W-CIO; l.

Forward copies of the transmittal to the individuals indicated on the form.

Copies of the documentation shall be provided to i~

transmittal recipients as indicated on the trans-mittal.

In cases where the documentation is voluminous (use judgement, normally 5 or more pages) copies of the documentation do not need to be provided to any transmittal recipients.

Documentation Not Supplied At The Time Of Request The MPQAD individual requested to supply the documentation shall:

a.

Complete the S&W-CIO Documentation Transmittal (Attachment A);

b.

Obtain the signature of the appropriate Section Head or-higher; IC0384-0001A-QLO2

c.

Forward the original of the transmittal with copies of the documentation to S&W-CIO in accordance with their request; d.

Forward copies of the transmittal to the individuals indicated on the form.

NOTE:

Copies of the documentation shall be provided transmittal recipients as indicated on the trans-mittal. In cases where the documentation is voluminous (use judgement, normally 5 or more pages) copies of the documentation do not need to be provided to any transmittal recipients.

Controlled Documents 10r Documentation for which S&W-CIO Requests To Be Placed On Routine Distribution The MPQAD individual requested to supply the controlled documents or documentation shall follow item 1 or 2 above for the first submit-tal. The transmittal shall indicate that S&W-CIO is being placed on routine distribution. Th,reafter the information shall be provided by memo, document transmittal / acknowledgement, or other appropriate means.- Individuals indicated as a transmittal recipient for the first transmittal do not have to be notified of the following

. transmittals.

The Transmittal Form (Attachment A) is for the most part self explanatory. The transm1ttal number shall be obtained from the

-DCC in the MPQAD trailers (phone x6382), and the file copy shall be forwarded to the DCC in the MPQAD trailers (attention CMThompson.)
