Leaking Check Valves

Dear Mr. Gagliardo:

This letter provides an interim report on a potentia L 10CFR50.55(e) concerning leaking check valves at Wolf Creek Generating Station.

This matter was initially reported by Mr. Otto Maynard of Kansas Gas and Electric Company (KG&E) to Mr. Michael Murphy of the Nuclear Regulatory Commission, Regien IV, on February 6,1984.

While filling the primary system it was noted that a check valve in the high pressure coolant injection (EM) system was leaking.

In-vestigation revealed that the valve had been disassembled and since a new gasket was not available, the bonnet bolts had only been " snug" tightened until the valve could be reassembled with a new gasket. The existing condition of the valve was documented on its traveler, but was not identified on the Turnover Exception List. A doctunentation review was initiated to detcrmine if other valves could have been affected in a similar manner. Documentation packages for approximately nine hundred valves were reviewed. This review revealed a total of nine valves with incomplete work not previously identified on the Turn-over Exception List. Seven of the nine valves were awaiting new gaskets for reassembly and the other two had the internals removed to support flushing activities. Although Special Instruction Sheets for these valves contained the correct information, this information was not properly transferred to the Turnover Exception List.

An investigation to determine the cause of these discrepancies resulted in the review and revision of procedures, ensuring that adequate measures exist to identify incomplete work. Other travelers addressing similar items are currently being reviewed for completeness. Any incomplete work and/or documentation identified will be listed on the appropriate Turnover Exception List.
