Waterford 3 SES USNRC Inspection Report 50-382/83-17 In accordance with the provisions of Section 2.201 of the NRCs " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, the following supplemental information regarding the Notice of Violation identified in Inspection Report 50-382/83-17 dated June 6, 1983 is hereby submitted:
VIOLATION 1
" Failure to Provide Required Safety-Related Records in Accordance With Established Procedures" 10 CFR 50, Appendix B, Criterion V, states, in part, " Activities affecting quality chall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."
Ebasco Services, Inc. (Ebasco) Specification 860-75, "QA Requiremer.ts for Suppliers of Safety-Related Equipment and Services," paragraph 4.17, QA Records, states, in part, " Sufficient records shall be prepared as work is performed to furnish documentary evidence of the quality of items and of activities affecting quality.
. The records shall include the results of reviews, inspections, tests, audits, monitoring or work performance.... Required records shall be iden ifiable and retrievable..."
American Bridge Procedure 23, Revision B, October 4, 1979, "QA Documentation Turnover," paragraph 6.4.1, states, in part,
" Implementation of this procedure shall be initiated in adequate time before completion of site activities to permit Ebasco's review and determination of acceptability of records.

American Bridge. Specifically.-Ebasco failed to provide adequate QA documentation for the. erection of structural steel for the missile protection over;the east and west main steam isolation valves.

' Response TheLfollowing supplemental information is provided for Violation 83-17-01:
Corrective Action:
Due to the lack of documentation being available for the erection of structural steel for the missile protection over the East / West Main Steam Isolation Valve..Ebasco Nonconformance Report No.-5594 was initiated. The 4
evaluation by TSSE was (1) to have = Ebasco Quality Control perform a complete reinspection of all bolted connections shown on drawing G-842-B in accordance with the requirements of procedure' ASP-IV-129,fand (2) that a complete Engineering evaluation be made of all welded connections shown on drawing C-842-B=in accordance with ASP-IV-128.
The reinspection of' bolted connections,is in progress. The results of the
. reinspection of the bolted connections and the Engineering evaluation of the

welded connections wil1 be documented.
Date Of Full' Implementation::
LResearch into the status of the welded connections is in process. We project a closure 'date of February-15,'1984, for NCR 5594.
VIOLATION >3
" Failure to Establish' Adequate Document-Control Measures" 10CFR50 Appendix B, Criterion VI requires that measures be established to control the issuance of documents.
.ANS'I-N45.2, paragraph 7, " Document Control," requires that procedures be established for the control of documents and changes thereto to preclude the
- possibility of use of outdated or inappropriate documents.

-The following supplement'al information is provided for Violation 83-17-03:
Ebasco New York EngineeringLhas reviewed approximately 10,000 drawings to determine:which.ones-have forced revision that may have design changes that are not incorporated into the drawings, and found that there are approximately L182 drawings that_are still in the' review process. This review is scheduled ito be completed on February 29, 1984.
