Dear Mr. Keppler:
INTERIM 10 CFR 50.53 (e) REPORT NO. U-69 CALLAWAY PLANT On February 7, 1984, Union Electric reported a 50.55 (e) significant deficiency to Mr. Paul Pelke, NRC Region III.
The deficiency involves missing seismic positioners on all four breakers used in Callaway Unit No. 1 Reactor Trip Switchgear.
These breakers were supplied by Westinghouse as part of the NSSS package.
Westinghouse has stated these breakers are seismically qualified with the-seismic positioners installed.
Thus, the omission of the positioners voids the assurance that the breakers would operate safely during a seismic event.
These breakers perform the safety-related function of interrupting power to the control rod coils, which allow the control rods to shut down the reactor via gravitational pull.
Westinghouse has determined that the lack of seismic positioners on the four DS-416 breakers was due to an oversight on the part of a Switchgear Customer Order Engineer who did not properly fill out zul order entry form on that one shop order.
As a result, the manufacturing information did not call for the seismic positioners.
In checking their shop order for Callaway Unit No.
2, which do have seismic positioners installed, they l
found that the same Customer Order Engineer had correctly filled out the order form and the manufacturing information called for the positioners.
Therefore, Westinghouse has concluded that this l
was a one-time event not likely to be repeated.
To confirm this, l
Westinghouse has indicated that they will make a random check of 15-20% of other Reactor Trip Switchgear orders they have supplied.
Westinghouse is currently in the process of providing seismic positioners for installation on the Callaway Unit 1 breakers.
