Dear Mr. Voltaggio:

Enclosed is the completed spill questionnaire (No. VA-84-001 - 2/13/84 North Anna Power Station) for the oil spill that occurred at our North Anna Power Station on February 13, 1984 As is noted in the questionnaire, an oil sheen was discovered by station personnel on the af ternoon of February 13, near the condenser cooling water intake structure on Lake Anna. The sheen was small in size and was determined to have been caused by lube oil associated with an overflow of the bearing cooling tower make up reservoir.

The oil was cor.tained by a floating boom and was removed from the lake surface using absorbent materials. Due to the small amount of oil involved and quick cleanup action by station personnel, no adverse impact to the environ;aent was observed.

The level control inlet i,alve for the bearina coolina tower was inadvertently left in the manual-open position.

Furthermore the level control outlet valve and a level indicator and an alarm were found to
