. Technical Specification 4.6.B.2.C requirec, in part, that a monthly proportional cmposite sample of each lig. tid effluent discharge batch be
. analyzed for tritium.
. Contrary to the above, no monthly proportional composite sample for liquid effluent discharge batch 58-82, released on August 22, 1982, was analyzed for tritium. Also, no tritium assay was conducted on samples from the two liquid effluent discharge batches released during March 1982.
Wis is a Severity Iavel V violation'(Supplement IV).
' Reply:
We concur ith the violation as stated A review of all Chemistry-related surveillances required by Appendix A Technical Specifications was performed for the period January 1982 through September 1983.. No discrepancies in effluent records during the period in question were identified in addition to those already indicated in the Notice of Violation.
Procedure No. 832.1, "Chomistry Surveillance Test Program", was reviewed for coupleteness. Revisions to the above procedure ensure that all Appendix A Technical Specification Chemistry surveillances are listed and coupleted as scheduled. Specifically, weekly review of surveillances and
. sign-off documentation is now required by Procedure No. 832.1.
Supervisor awareness has been increased by the weekly surveillance follow-up and sign-off requirement. Furthermore, periodic in-depth review by an independent staff member is also conducted.
' he corrective steps discussed above have been taken to prevent a recurrence of this type.
Ee. data review was completed on October 26, 1983. We procedure was revised and full implementation was achieved on November 21, 1983.
