During the initial fuel loading operation on 8/9/73, the refueling interlocks were found to be inoperable. This event was reported to Mr. Tom Shedlowksy, A.E.C. Eegicn 1 Regulatory Operation Office, on 8/10/73 In accordance

with Section 6.7.2.A of the Technical Specification Appendix A of DPR-S for Unit #2 Peach Botton Atcmic Power Station, this occurrince is being reported to the Directorate of Licensing as an Abnormal Occurrence.
Prior to the start of initial fuel loading operations on Unit #2, the refueling interlock surveillance test ST 12.1 was performed. This test was successfully completed on 8/8/73 on afternoon shift. Actual fuel loading operations were started on 6/9/73 at 7:55 p.m.
After loading the first seven elements, the reactor operator noted that the rod block annunciator was not actuated each time the refueling platform was moved over the reactor vessel. Fuel loading operations were immediately halted to investigate.
Investigation The investigation of this failure revealed that one of the refueling plat-form bridge travel position switches had failed. The second switch also showed signs of distress. Switch failure was~ caused by a mechanical inter-ference problem between the switch and a portion of the track on which it rides. Subseouent discussions with several reactor operators concerning the rod block annunciator indicated that the switch failure probably occurred during a fuel loading training exercise performed shortly after the refueling interlock surveillance test was completed.
