Notice of Violation Inspection Report 83-35 The subject Inspection Report, dated December 23, 1983, identified a violation concerning failure to follow procedures.

- Based on the results'of an NRC inspection conducted during the period of November 1-30, 1983, and in accordance with the NRC Enforcement Policy (10 CFR Part_2, Appendix C), 47 FR 9987, dated March 9, 1982,. the following: violation was identified:

Failure to Follow Procedures Technical Specification 5.8.1 requires that " written pro-cedures.-.. be established, implemented, and maintained that meet or exceed the minimum requirements of Section 5.1 and 5.3 of ANSI N18.7-1972, and Appendix A of USNRC Regu-latory Guide 1.33 Standing Order G-7 establishes the Operating Manual and states in Section 1.3 that " adherence to the Operating Manual is mandatory."

Standing Order G-21, " Station Modification Control," es-tablishes-instructions for initiating, controlling, and documenting actions pertaining to modifications of station equipment and facilities at the Fort Calhoun Station.

Section 5.7.6 states that during post-installation checkout, the planner signs the Post-Installation Modification Control

- Form to indicate that revisions to all modification plan-ning documents have been properly reviewed and approved."

- Contrary to the abnve, the revies and approval steps of CQE Piping Isometric D-4238, Sheet 4 of 7 were improperly done in that a hydraulic anubber was incorrectly shown on the

' drawing and. identified in the data table when the device was no longer required or installed.

This is a' Severity Level IV Violation.

The District has reviewed.the proce.ss by which enubbers are deleted and added to systems.-

This review resulted in identifying the need to-implement corrective actions as

- discussed in section-(2.) of this report.

In addition, draw-

-ing D-4238,. sheet 4, has been revised.

. (a)

The list of snubbers that have been removed or re-placed with rigid restraints will be rechecked against the applicable subsystem drawings to ensure that the type of restraint is listed correctly in the load tables.

Revisions will be made as necessary.

(b)

The T-pipe models of subsystem FW-289A and any other subsystems found as a result of (a) will be changed to reflect the proper restraint.

(c)

-Hard-copy T-pipe computer runs will be annotated to reflect the change in the input modal.

(d)

The GSE Mechanical Department will develop a checklist to be completed prior to requesting SARC approval of a proposed modification.

One item on the checklist will be "All drawings that will be affected by this modifi-cation have been listed as requiring updating."

(e)

The appropriate steps to be taken when dealing with snubber removal will be re-emphasized to GSE design enginoces and GSE draftspeople.

The date when full compliance will be achieved.

The District will be in full compliance by March 1, 1984.
