I UNIT 1 REFUEUNG 18 INSERVICE INSPECTION 1.0 INTRODUCTIOR  !

From May 25,1990 through May 25,1991, nondestructive examinations of selected components at PBNP Unit I were performed. These examinations constitute the first examination of the first period of the third t0 year interval at PBNP Unit 1. Also included in this examination were items rema!ning from the second 10 year hterval. The time interval for this examination period included the Unit 1 Refueling 18 outage which ran from AprB 6,1991 to May 20,1991, 2.0 EXAMINATION AREAS 2.1 Determination of Scoce The ISI was performed in accordance with the following:

2.1.1 PBNP FFDSAR, Technical Specification Section 15.4.2.B 2.1.2 Long Term inservice Examination Plan (LTP) for Class 1. Class 2 and Class 3 Components and Systems of PBNP Unit 1 (Second Interva!)

2.1.3 ISI Long Term Plan for the Third inspection Interval, Unit 1 2.1.4 NRC IE Bulletin 82 02 2.1.5 Requests of PBNP Personnel 2.2 .N:'E Lkilnal Representative samples of the following components and system areas were examined with NDE techniques by WE (QAS), EBASCO, and Professional Welding Associates (PWA) peitonnel. For more detailed information see Unit 1 LTP, Second or Third inspection interval, as applicable.

All the examinrl'ons were performed in ac:ordance with written procedures that conform to the appi: Lab'c sections of the ASME BOui and Pressure Vessel Code. The following are summarlos tJ the recorded indt ations found uttring the examinations and evaluated by PBNP personnel.

i 3.1 IDR 7 P 001, Class 3 Pipe Support, S 541 The as-btP configuration of the support is different than shown on the support drawing.

This is nonrelevant per ASME Sec'lon XI, IWF-3410(b) and was eccepted for continued ut-e. A Condition Report was in!tlated to evaluate the support / drawing end make changes as appropriate.

3.2 IDR 7-P-002, dass 1 Pressure Retalning DissimHar Matal Weld, Pressurizer Safety Nozzle, PZR SAFNOZ 1.

Numerous dye penetrant Indict ;lons (s!zes 1/16' 1/8*) were kjentified in the area of the nozzle to safe-end weld. A!! of the indications were less than the acceptance limit l of Tablo IWB 3514 2 o' the ASME Section XI Code.

Numsrous dye penetrant indications (t,12es 1/16' . 6/10") were identified in the area of the noz21e to safo-end weld and adjacent base metal. The Indications in the ASME Section XI examination area were less than allowabte by Table IWB 3514 2 of the Code.

The base metal irdications were reexamined and enluated in accordance with the original construction requirements (ASME Section 111,1065 edition, Summer 1906 eddenda). The base metal Indications were also acceptable.

3.4 IDR 7 P404, Oms 1 Pressure Retalning Botting Steam Generator 'A' Ou'!et Manwi

  • Botts, TG A-MY/AY OUTLET BLT 01 through 16 One bott had a gouge in the mating surface of bott head. The gouge was evaluated as accepta!9e per.!W34517 of ASME Section XI, but the bott was replaced as a precautionary measure.

3.6 108 7 P406, dass 2 Pipe Support FW H12 The load indicator of the spring can was reported ru;.bing against the sides of the indicator slot and a tack weld at the base of the e,anl ster was cracked. The cracked tack weld was first identified in 1908 and evaluated as acceptable without repair. The crack remains unchanged and acceptable. The Indicator is a nonload carrying part and is therefore acceptable as is.

3.7 IDR 7-P407, Cass 2 Piping, SIS 10-SI 100549 Two dye penetrant Indications (stzes 3/32' and 1/8') were reported in the weld area.

These are less than allowable per Tab!s !WB 3514-2 of ASME Section XI.

3.8 IDR 7-P408, Cass 3 Pipe Support, R 206 The as-bu!!! configuration of the pipe support is different than shown on the support drawing. This la nonrelevard per ASME Section XI, IWF 3410(b), and was accepted for continued service. A Condition Report was inttlated to evaluate the support / drawing i- and make appropriate changes.

! 3.9 IDR 7 P 009, Oass 2 Pipe Support,81-50 Loose botting was reported on the pipe support. This condition did not meet the acceptance criteria of ASME Section XI, IWF-3410(a). MWR 912116 was initiated to

, tighten the botting and a Condition Report inttlated to evaluate other supports on this l system. The unacceptable condition also required the examination of seven (7) l additional supports of similar type, design av hnetion.

3.17 IDR 7 P-017, Oass 2 Pipe Support. MSH 06 The portion oP ne support rod which extends above the spring canister was reported as bent. This pn'tlon of the rod does not carry load and was evaluated as a nonsupport part. The st p art was accepted as is for continued service.

U1R18 Inservice Inspection Page6 3.18 IDR 7 P 018, Cass 2 Pipe Support, St-04 The inspection reported that the U bolt was loose and the corresponding nuts were not snug. The condition was unacceptable per ASME Section XI. MWR 912257 was issued to correct the support. This support was examined as part of tho evaluation of Condition Report 91091.

3,19 IDR 7 P 019, Qais 2 Pipe Support Sl45 The inspection reported that a nut on the support rod was loose. The evaluation determined the nut in question was not required for the function of the support and had been backed off during erection. The condition was acceptable as is. This support was examined as part of the evaluation of Cordition Report 91091.

3.20 IDR 7 P420, Oass 2 Pipe Support, SI-66 The inspection reported the U bolt was loose and the corresponding nuts were not snug. The conchion was unacceptable per ASME Section XI. MWR 912257 was issued to correct the support. This support was examined as part of the evaluation of Condition Report 91-091.

3.21 IDR 7 P-021, Cass 2 Pipe Support, SI 67 The nut was reported missing from the adjestable clevis cross bolt. This condition did not meet the acceptance criteria of ASME Section XI, lWF 3410(a). MWR 912257 was issued to replace the missing nut. This support was examined as part of the Condition Report 91091 evaluation.

3.22 IDR 7 P 022, Cass 2 Pipe Support, SI-69 The support was found not to be attached to the building structure. The evaluation of the support Indicated it was not required and should be removed from the pipe. The removal was included on MWR 912257. This support was exam'ned as part of Condition Report 91-091 evaluation.

3.23 IDR 7 P 023, Cass 2 Pipe Support, SI-72 The inspection identifled U-bolt nuts which were not snug. MWR D12257 was issued to '

have the nuts tightened. This support was examined as part of the Condition Report 91491 evaluation.

3.24 IDR 7 P 024, Cass 2 Pipe Support, SI 75 This support was found to be, and dispositioned t'.e same as, IDR 7 P-023.

3.25 IDR 7-P 025, Oass 2 Pipe Support, SI 81 The nut on the cross bolt of the adjustable clevis was reported as not tight. This was evaluated as a construction deviation. Tightening of the nut was included on MWR 912257. This support was examined as part of the evaluation of Condition Report 91091.

4 U1R10 inservice Inspection Page 7 3.26 IDR 7-P 026, Oass 2 Pipe Support SI 85 The nut was reported missing from the adjustable clevis cross bolt and the hanger rod bent at 45 degrees at its upper connection. The bent hanger rod was per design; however, the missing nut did not meet the acceptance crherla of ASME Section XI, IWF-3410(a). MWR 912257 was issued to replace the missing nut. This support was examined as part of the Condition Report 91091 evaluation.

3 27 IDR 7 P 027, Cass 2 Pipe Support, SI 79 This inspection reported the top anchor bolt on this U-bolt support was lost. This condition is unacceptable per ASME Section XI. MWR 912211 was issued to repair this condition. Condition Report 91112 was initiated to conduct an evaluation of this condition. This condition requires repair prior to startup and is scheduled as such. No additional exams were required since all supports of this size, type and function have been examined this outage.

3.28 IDR 7 P 028, Class 2 Pipe Support, SI 51 The nut on the cross bott of the adjustable clevis was reported as not tight. This was evaluated as acceptable per ASME Section XI since the nut was immovable and meets the general assembly requirements of Grinnell.

3.29 IDR 7-P 029, Class 2 Pipe Support, S127 This inspection reported an anchor bolt missing. This was evaluated as a construction / modification deviation and not service related, thus meeting ASME Section XI acceptance standards. Condition Report 91108 was initiated to evaluato the support and make corrections to the as-bul!! drawing as necesury.

3.30 IDR 7 P 030, Class 2 Pipe Support, SI 13 This inspection reported that the support does not meet the drawing requirements. This discrepancy is not a degradation of the support structure and is acceptable per ASME Section XI. Condition Report 91107 was initiated to evaluate and change the file drawing to reflect as built configurations.

3.31 IDR 7 P 031, Class 2 Pipe Support, SI 39 This support was found to be, and dispositioned the same as, IDR 7 P 030.

3.32 IDH 7 P 032, Class 2 Pipe Support, Sl 16 This support was found to be the same as IDR 7 P 030. Condition Report 91106 was initiated to evaluate and chcnge the file drawing to reflect the as bullt configuration.

3.33 IDR 7 P 033, Class 2 Pipe Support, SI 41 This support was found to be, and dispositioned the same as, IDR 7 P 032, 3.34 CR 91091 A Condition Report was issued to perform Preservice or Baseline examinations on fourteen (14) supports in the Safety injection System. All of these supports were added to the ISI Program for the third Inspection in'erval and had not been previously examined for ISI.
