Butterfly Valves
Dear Mr. Gagliardo:
This letter provides an interim report on a potential 10CFR50.55(e) concerning Fisher 9200 series butterfly valves installed at Wolf Creek Generating Station. This matter was initially reported by Mr. Otto Maynard of Kansas Gas and Electric Company (KG&E) to Mr. William Johnson of the Nuclear Regulatory Commission, Region IV, on January 10, 1984.
During flushing operations KG&E Startup personnel experienced difficulty in operating a twenty-inch butterfly valve located in the component cooling water (EG) system. Subsequently the valve internals were removed and examined to deternin' the cause of the operating difficulties. At this time it was discoverud that the two tapered pins which secure the butterfly disc to the valve shaft were missing. Upon boroscopic examination of the surrounding pipe, one of the two tapered pins was located and removed. The disc on this type of butterfly valve is fixed to the valve shaf t with two tapered pins four inches in length. Eact pin has an outside diameter of 0.591 inches and tapers at a rate of 0.25 inches per foot.
During assembly these pins are press fit into position.
The valve in question is a Fisher Control Company, Type 9270, twenty-inch butterfly valve, Mark #20-HBC-WBF-Q-F-C.
Another valve of this type, also in the EG system, was disassembled and upon examination it was discovered that although this valve was operating properly, one of its tapered pins was not in place.
An investigation is being conducted to determine the scope of this problem.
As part of this investigation, Fisher Control C mpany has been contacted to assist in evaluation and resolution of this condition.
