Conditions Prior to occurrence:
Unit 2%at 90'; power.
D;ccription of Occurrence:
During local leak rate testing of the Standby Liquid Control System pcactration, a nor:. ally locked open block valve was closed to acco=plish the test.
Following the test, the valvo was Icf t closed and the plant was subsequently acarted up.
Designation of Apparent Cause of Occurrence:
In order to provide a valid test, the Renults Engineer approved a change to the valve lineup pe':-J t ring the packing to be loosened on the subject valvo no as to provide a veat up :tream of the cheek volve which forms one of the icolation valves.
During t'a? t e.st setup, the individual anninned to the test apparently closed the valve to providd a vent path through the packing.
Although+ao violated the proceduru by cloning the valse. he did hang a leak rate tout can on it. When the test vao conpleted, the valve packi33 van retfuhtoned,
- .' b u t the valve was not reopened, locked, or untagged.
Following reactor startup, the tag was observed by a aember of the plant utaff and subsequent investigation revealed this reported occurrence.
Analysis of Occurrence:
Closure of the black valve c:ade it inoo n iM c to infect the Standby Liquid. Control Synten into t.ha reactor.
However, in tha wllk21'/ event that inj ection vaa required, the fafluro of the nynten to inject would have been obaarved by the reactor operator due to the t'act that the Standby Liquid Control Tank level vould not decrease.
Subsequent inventigation would have renedied the situation.

Perconnel. involved in Local Leak nace tenting have been instructed in the proper ut:tbod of performing the tanto and r..sking test procedure changen.
A reviced LLnr procedure will hw Ponc revicked and approved, and pill be it.plemented for future testing.
