j Description of Occurrence:
i During a routine instrument surveillance test, the setpoint i
of the condenser low vacuum scram switch, PS-2-5-llA, was found to
' be outside the limit of greater than or equal to 23 in. IIg.
The' device t
performed satisfactorily.

Designation of Apparent Cause of Occurrence:
This occurrence is due to setpo' int drift of the pressure
[
t
-switch.

t Analysis'of Occurrence:
- The contacts of this device are arranged in a one-out-of-two' t
twice trip logic.
The setpoint of.this device, not the trip function, was unsatisfactory.
All other devices monitoring the parameters of condenser vacuum were immediately tested and found to be operational and correctly calibrated.
Because of the above reasons this occurrence has minimal safety implications.
j Corrective' Action:
,Li The out-of-tolerance' pressure switch was immediately re-calibrated to within the allowable limits of the technical specifications.
The other three devices monitoring the same variable were tested
.and found to be satisfactory in both function ano setpoint.
pm Failure Data:

^Two occurrences of this type have been previously reported as AO-50-277-74-1 and AO-50-277-74-19.
After the latter, the device
.was r'eplaced in kind.
A calibration shift was again reported in AO-50-277-74-35.
