Dear Mr. Giambusso:
Subject:
Environmental Deviation The following deviation was reported to Mr. Phil Stohr,
"~
A.E.C. Region 1 Regulatory Operations Of fice on November 29, 1974.
In accordance with Section 7 4.2.A of the Environmental Techni cal Specifications, Appendix B of DPR 44/56 for units 2 and 3 Peach Bottom Atomic Power Station, the f ollowing report is being submitted to the Directorate of Licensing as an Environmental Deviation.

cA Environmental Deviation Report //74-5 indicated the f ailure of

- Q the 'B' cooling tower lif t pump on November 4,1974. Due to the nature g7//
of the damage, that cooling tower remains out of service at this time.
9 On 11/22/74, the 'C' f an motor on the 'C' cooling tower f ailed. The h
iw[
dif ficulty was traced to failure of the motor bearings and possibly a l/
motor stator problem. Technical Specification paragraph 2.1 considers the cooling tower operabic with 10 of the 11 installed fans operating.
The.'C' f an motor was removed and taken of f site for repair.
On 11/29/74, the 'F' fan'on the
'C cooling tc,.cr failed. This reduced the operating y/
fans on the 'C' cooling t ower' to 9, thereby causing it to be corg' dur-g gg M 1**d-la h Tochaical-_Soccificatiom River fIew

Mr. A. Giambasso December 9, 1974 Page 2 Conditions Prior to Deviation:
Unit 2 operating at 100% power for the first 12 hours1.388889e-4 days
0.00333 hours
1.984127e-5 weeks
4.566e-6 months
during the time two f ans on the 'C' cooling tower were inoperable. Unit 2 was subsequently shutdown for other reasons late on 11/29/74. Unit 3 was not in operation during this interval.
Description of Deviation:
Investigation of the failure of the 'F ' f an on the
'C' cooling tower has indicated a f ailure of the shaf t between the motor and the gear box or inside the gear box. As indicated, the failure of the 'C' fan on the 'C' cooling tower was caused by motor bearing and motor stator difficultics.
Ocsignation of Apparent Cause of Ocviation:
Not determined at this time. Engineering Depar t.T.cnt i s in-vestigating.
Analysis of the Deviation:
Because of the high river flows, and the fact that temperature distributions did not vary significantly, no adverse ef fects to plankton and fishes is anticipated.
Corrective Action:
In order to return at least one of the f ailed f ans on the 'C' cooling tower to an operabic condition, the motor from the
'F' fan was relocated to the
'C' fan on the afternoon of 11/30/74. Ten of the cleven cooling toacr fans were back in service by late on 11/30/74, thus re-turning the 'C ' cooling tower to an operable condition as defined in the Technical Specifications. Work is continuing on repair operations on the 'F ' fan.
F ai lure Dat_a_:
Similar environmental deviations were reported in Environmental Deviation Repor ts #50-277/278-74-4 and 50-277/278-74-5 Very truly yours, p),,j1
