ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNITS 1, 2, & 3 I.

Description of Deficiency While performing hydro-static testing at PVNGS Unit 2, motor operated valve (MOV) 2J-CHA-HV-524 was manually closed to the extent that the 5/16" mounting bolts, fastening the actuator to the valve yoke, failed.. Review of vendor documentation indicated that the valve maximum allowable rim pull is 40 lbs.

It has been determined that during testing the maximum 40 lb. pull requirement was not exceeded. The valve is supplied by Combustion Engineering (C-E) and maaufactured by Borg-Warner.

C-E's investigation indicated that this problem is applicable to Borg-Warner part Nos. 77610 and 83110. The corresponding PVNGS valve tag numbers are Units 1 and 2 CH-524 (2 inch) and Units 1 I

and 2 CH-255 (1 1/2 inch).

II.

Analysis of Safety Implications Valve CH-255 is installed as an Outside Containment isolation valve for the Reactor Coolant Pump (RCP) seal injection line. Valve CH-524 is installed as Outside Containment isolation valve for the

. charging pump line.

Both valves have safety-related functions as defined by C-E letter V-CE-19011.

Letter V-CE-19011 attributes the~ mounting bolt failure to inadequate bolt material selection and an insufficient number of mounting bolts required to adequately secure the motor operator to _ the valve yoke assembly and satisfy the 40 lb. rim pull requirement.

In addition Borg-Warner has indicated that damage to the valve also might have

occurred if'a locked rotor motor operation of the valve had l,

occurred. However, C-E's specification No. 00000-PE-707, Rev. 2

requires that no damage to the valve shall be sustained by this event. Therefore, Borg-Warner will provide redesigned valve stems l

and rims as required, to preclude damage to the pressure retaining l

parts due to manual or electric motor overtorquing events.

I Based on the above, this condition is evaluated as reportable under the requirements of 10CFR50.55(e) as a significant deviation from l

the design specifications, and if left uncorrected, this deficiency would represent a significant safety condition. The PVNGS project also evaluates this condition to be reportable under the requirements of 10CFR Part 21.

Deficiency Report 83-46 addresses j

the reporting requirements of 10CFR Part 21.21(b) (3), with the exception of sub-part (vi) which requires the number and location of other possible defective components, l

C-E letter V-CE-20369, recommends as an interim solutio.3.that the subject valves be limited to less than 25 lbs. rim p'.tll, until the problem is fully corrected.

B.

C-E letter V-CE-19199 provides the Borg-Warner Field Rework Prodedure (Report No. 2023) for modification to the identified Unit 1 and 2 valves.. Bechtel will implement the following

~ modifications, before first fuel load, via Design Change Packages (DCP's) ISM-CH-163 and 2SM-CH-163 as follows:

Replace existing moun' ing bolts with ASTM A193, Grade B7.

c Modify valve yoke, by drilling two new holes, to accept 1/2 inch ASTM A193, Grade B7 bolts.

Install Borg-Warner stem and disc field kit.

C.

NCR PA-6267 will be dispositioned as per A and B above.

D.

C-E has stated that Unit 3 valves have not been delivered to PVNGS and that modifications will be performed by Borg-Warner prior to delivery.

E.

C-E will submit revised drawings, manuals and documentation related to the Borg-Warner valve modifications.

F.

A copy of this report will be sent to C-E for their review and disposition in accordance with - the requirements of 10CFR21.
