3 A Grid Fault caused a False Sudden Pressure Relay indication due to an electro-mechanical shock.
This initiated a Turbine Trip, Reactor Power Cutback, and a transfer of House Loads from the Unit Aux Transformer (UAT) to the Start Up Transformer (SVT). However the UAT breakers were slow to open and the SVT breakers closed, reconnecting the Generator to grid.
The generator output (while dropping) went out of phase. The Reactor Coolant Pumps experienced a voltage drop, slowing down, and the Reactor Protection System triped the plant on low Departure From Nucleate Boiling Ratio.
The VAT breaker, whose interruptable rating had been exceeded, failed internally as it attempted to trip, creating ionizing gases which promoted electrical arcing, starting a fire in the Switchgear room.
