(1) Please provide details pertinent to safety-related pipe failures due to freeze induced bursts.

Date of pipe bursts.

(b)

Lowest temperature re bursts.

(c)

Number of bursts.

Length of cracks.

(e) size of peices burst off pipe.

(f)

Number of bursts occuring at welds.

(g)

Number of bursts occuring within 3,6,9,12 inches of any welds.

(h)

Provide test method used or to be used to check stresses l

I at other locations where bursting did not occur (i)

Give number of feet of pipes of all sizes involved.

(j)

Number of tests for stress made on pipes in (i).

(k)

Number of bursts or cracks that occurred at welds.
