During a routine inspection of the Unit 3 hydraulic snubber during a plant L;l caintenance outage, hydraulic onubber 3-1 0-113-5-7 was observed to have becoce M.Yd -
inverted resulting in the draining of all hydraulic fluid out of the reseWoir vent.

Designation of Apparent Cause of Occurrence:

Preliminary investigation indicates that the lock nut which prevents rotation of the anubber cechanima was not tightened properly.
I Analysis of Occurrence:
The defective snubber is installed on the discharge piping of the "B" RHR pump for seismic support only.
It serves no f unction during nortal plant operations.
The g losu of fluid from this snubber has minical safety significance.
Corrective Action:
The anubber is being returned to service in the proper position with the lock nut properly tightened. Additional investigation is in progress.
