Abnormal Occurrence The following occurrence was reported to Mr. Walt Baunack, A.E.C. Region 1 Regulatory Operations Office on September 19, 1974.
Written notification was made to Mr. James P. O'Reilly, Region i Regulatory Operations Office on September 19, 1974.
In accordance with Section 4.7.2.A of the Technical Specifications, Appendix A of DPR-56 for Unit 3 Peach Bottom Atomic Power Station, the following report is being submitted to the Directorate of Licensing as an Abnormal Occurrence.

Description of Occurrence:
During a routine inspection of the Unit 3 hydraulic snubber during a plant maintenance outage, hydraulic snubber 3-lO-HB-5-7 was observed to have become inverted resulting in the draining of all hydraulic fluid out of the reservoir vent.
Designation of Apparent Cause of Occurrence:
Investigation indicates that the locknut which prevents rotation of the snubber mechanism was not tightened properly.
Analysis of Occurrence:
The defective snubber is installed on the discharge piping of the "B" RHR pump for seismic support only.
It serves no function during normal plant operations.
Corrective Action:
The snubber was correctly positioned and the locknut adequately tightened.
The reservoir was filled to the correct level and the piston was stroked several times to remove all air from the hydraulic system.
All similar snubbers, which were readily accessible (approximately 525) were inspected and found i
to be tight.
Failure Data:
There is no record of a similar failure.
