HPCI isolation following automatic initiation. .

Conditions Prior to Occurrence:

Unit 3 reactor isolated and scrassmed as part of a startup test program.

DOccription of Occurrence:

During the review of data taken during the 100% reactor isolation test performed on 11/19/74, it was noted that the HPCI had received an initiation signal during the transient. The HPCI then icnediately isolated. The HPCI was then reset and operated nanually to inject water int D30ignation of Apparent Cause or;o the reactor and prove operability.

Occurrence:

A review of the test data and interviews with the operators present during the transient has not provided an absolute cause.

Analysis of Occurrence:

During this transient, the reactor water level momentarily dropped to the initiation level for EPCI and RCIC. Reactor water level was immediately recovered by way of the r: actor feedpump. HPCI operation was not required during this trancient. All other engineered safeguard systems operated as expected. This occurrence has minimal safety implications. .

Corrcetive Action:

In an attempt to further investigata and determine the exact cause of this occurrence o EPCI quick start with a condensate storage tank to condensate storage tank flow path is scheduled for 11/26/74. A vessel injection test will be repeated prior to completion of a 100% testing program to further ensure EPCI operability.
