Linitorque cotor cupport on liO-10-154B vibrated loose causinx twtor to fall off limiterque cperater.

caignation of Apparent Cause of Occurrence:

111gh vibration during operatim of this valve couned cap acrews which hold the cotor to cu cdapter plato to loopen.

tanlycic of Occurrenco:

Thio occurrenec' causco the valve to fail in the "ac-lo" position.

During shutdown coolt-operctions, the valve can be positioned nanually to provide proper nhutdown cooling flow rato.

During reactor operation this valve io normally in the full open pooitton.

High vibrction la only prosent when valve is operating in a throttling trodo.

Safety cignificance is ninien1 sinco the high vibration which cauood failure to not preocnt with the reactor in operation.

-Crrcctive Action:

Dausged nears cro being replaced.

The adapter plato 10 being repaired.

New bearingo fo' th2 cotor vill be provided.

The cator vill be refantened to the cdapter plato using nw ccp cerevs and LOC-TITE.

The reactor can be returned to norvice with the 154D valve in th3 full open position cince the valvo corvos no saf ety function.

iciluro Data Siciler fcilurco hevo occurred as reported in Abnormal Occurrence Report Numbern 50-277-74-53 and 50-270-74-28.
