Failure of LS-4221 to operate (RHR loop
'A' high point vent) on, low accumulator level.
Conidtions Prior to Occurrence:
Unit 2 at fulI power.

4 During routine local leak rate testing of RHR system valves, it was noted that the low level alarm did not operate and vent solenoid valve SV-4221 did not open.

Apparent binding in the switch assembly.
Ij Analysis of Occurrence:
l The safety implications are minimal since the system was found l
to be full at the start of the local Icak rate test.
Corrective Acticn:
the During the time the solenoid valve was out of service, high point vent was checked to ensure a full system. The pressurizing system remained in service during this time.
Investi]ation revealed caused that switch binding, along with misalignment of the switches, The switches were replaced and aligned to give the optimum the failure.
