Conditions Prior to Occurrence:

Unit 2 at full power.

During a routine instrument surveillance test, T S 12-()9 was found to trip at 215 F, which is a higher setpoint than its allow-able Iimit of 1400F.

Page 2 Designat ion of Apparent Cause of Occurrence:

The instrument is mounted on a local rack in an open hallway which has been heavily traveled due to pump maintenance work in the area. The setpoint knob adjustment protrudes from the front of the device and was apparently inadvertently moved.

Analysis of Occurrence:

Safety significance of this incident is minimal.

The function of this switch is to prevent damage to the demineralizer resin from high temperature by isolating the system when the demincr-a l i zer inlet water temperature reaches 1400F.

Correcti ve Action:

The switch was immediately reset and checked in accordance with the applicable calibration check type of surveillance test.

A full front protective cover will be installed over the setpoint knob to prevent it from being inadvertent ly moved.
