Mr'. A. Giambusso 50-277-75-36 May 28,1975 Page 2 Designation of Apparent Cause of Occurrence:

Under investigation.

Analysis of Occurrence:

'Under normal operating conditions M0-2663 is closed. This valve is downstream of M0-38A and M0-38B, both of which had previously been found to be leak tight. M0-38A and M0-388 receive an is~olation signal above 600 psig reactor pressure. M0-2663 is the long path re-circulatiori valve which dumps to the condenser.

Safety implications of this occurrence are minimal.

Corrective Action:

The valve has been disassembled for inspection and minor indications on the seating surfaces noted. A seat lapping tool is being fabricated for valve repair..The valve will be repaired, re-assembled and tested as soon as possible.

Failure Data:

I:

None.
