Failure of operator motor on RER system valve MO-2-10-154B.-
Conditions Prior to Occurrence:
Reactor shutdown and depressurized.
Description of. Occurrence;~---- - -"
During the use of the RER system in the shutdcun cooling mode, the 154B valve was-positioned canually from the control,roon,,to adjust flow.
During this operation, it was noted.that the lielt lights on the valve indicated an. abnormal condition..
An inspection of the valve revealed that the sten anti-rotation key had fallen-out, resulting in rotation of the sten.
This rotation without. valve translation.
upset the calibration of the operator.
Apparently.the stem anti-rotation key had become ja==cd between the anti-rotation arm and. the packing follower which resulted in c cessive 1 cad being applied to the cotor as it attempted to cave the stem.
Thi-in turn, caused the tsocor to feil electrically.
Dasignation (,f Apparent Cause of Occurrence:
Under investigation.
Analysis of Occurrence:
Safety significance of this incident is ninimal since the recetor.vas chutdown and depressurized.
Shutdos.n cooling van still availabic to the reactor thru the 'A'
.RuR loop and.if nuaded, the.154B valve could have been positioned manually from the l operator.
Corrective Action:
The, sten key and motor operator.will be.xcpAaced. pp. coon as possible.
